Module 1
Understanding and establishing Hospital at Home
In this module, you will be introduced to Hospital at Home as a model of care and what it means to provide hospital-level treatment in the patient’s own home. You will explore how Hospital at Home can be organised and implemented, and what needs to be considered when assessing who can receive care at home. The module also presents key frameworks and prerequisites that support safe and sustainable care delivery. Finally, you will consider collaboration as a fundamental prerequisite, both within the healthcare team and across organisational boundaries.
Module 2
Initiating care in the home
In this module, you will explore how Hospital at Home care is initiated and what needs to be considered when hospital-level treatment moves into the patient’s home. The module follows the patient’s pathway into Hospital at Home and highlights the importance of assessing the home as a care environment. You will also focus on the first encounter in the home, including how to establish trust, identify risks, and create a shared understanding of the care plan. The module addresses safety and security for both patients and healthcare professionals, and considers how family caregivers can support care while also having their own needs and limitations.
Module 3
Delivering care in the home
In this module, you will focus on what it means to deliver Hospital at Home care in everyday practice. You will explore how daily care is planned, coordinated, and adapted to the patient’s condition, home environment, and changing needs. The module highlights the importance of teamwork, communication, and relationships when care is provided across professional, organisational, and personal boundaries. You will also consider cultural competence and the role of patients and family members, including both the opportunities and challenges that arise when care takes place in the home. Finally, the module addresses discharge from a Hospital at Home unit and how to support a safe transition after the acute episode of care.
Module 4
Data-driven and technological clinical practice
In this module, you will explore how technology and data support clinical practice in Hospital at Home. The module introduces technology as a prerequisite for providing hospital-level care outside the hospital and considers how medical technology can be safely used in the patient’s home. You will examine how data is collected, interpreted, and applied in everyday clinical decision-making, while also reflecting on the limitations and uncertainties that may arise. The module also highlights the importance of reflective support and digital health literacy, both for healthcare professionals and for patients and family members involved in care.

Quality, patient safety, and development
In this module, you will focus on quality and patient safety in Hospital at Home, and how safe care can be developed and maintained in practice. You will explore responsibilities and roles, including how clear communication and shared understanding support safe workflows in the home. The module also addresses risks and healthcare-related harm, and how these may differ when care is delivered outside the hospital environment. Finally, you will consider how implementation and improvement work can support sustainable development of Hospital at Home services over time.
